阪神大地震时，近八成灾民是由当地民众组织营救的。吸取那次的教训，日本明确了政府和民间的救灾责任区分，即大地震时，消防队的优先任务是灭火，因此灾民们必须要先进行自救。
2013年，日本政府发布了新的地震灾情预测，若发生首都直下型地震或南海海沟大地震，受灾人群将多达数千万人。而在公共部门承认能力有限的情况下，日本就如何利用民间力量有效开展自救活动，进行了社区互助救援、居民检伤分类等系列讨论和摸索。
社区互助救援：活用手边物品充当搬运设施，将受困老人运送到安全地带。
居民检伤分类：居民参与灾民伤情判断，将亟待急救的重伤员送往医院，轻伤员则送至救护所。
压伤综合征：在地震中经过长时间挤压的部位会产生毒素，毒素蔓延至全身将导致死亡，这是一种重伤，也是检伤分类要做的第一步，询问伤员是否受过长时间挤压，确认后需马上送往医院。
